About this subject
On successful completion you will be able to:
- Identify and discuss the basic principles of the Case Management process, including its application in different cultural groups and in diverse communities.
- Apply, demonstrate and report Case Management principles in relation to local, national and international practical case studies.
- Critique and interpret major models relevant to Rehabilitation Case Management and apply to practical settings.
- Analyse theoretical models and determine the most appropriate assessment tools to create and evaluate a rehabilitation intervention plan.
- Work collaboratively to demonstrate effective and innovative rehabilitative strategies and return to work planning, injury management and service provision

In this subject, you will be introduced to major concepts and theories in rehabilitation and health intervention. This subject will examine procedures that are useful in rehabilitation when working in health settings. The subject will provide students with an understanding of effective case management skills which leads to proper care coordination, seamless service provider integration and cooperation, and the development and delivery of comprehensive rehabilitation services to those living with chronic conditions or permanent impairments with the aim of achieving optimal health.
- One Viva Voce in-class activity (300-words equivalent) (20%)
- One individual written assignment (1,500-words). (30%)
- One 20-minute group presentation with 4-5 students (1000-words equivalent). (20%)
- One 60-minute test (1,000-words equivalent) (30%)

What’s a single subject?
Single subjects are the individual components that make up a degree. With Open Universities Australia, you’re able to study many of them as stand-alone subjects, including postgraduate single subjects, without having to commit to a degree.
Each of your subjects will be held over the course of a study term, and they’ll usually require 10 to 12 hours of study each week. Subjects are identified by a title and a code, for example, Developmental Psychology, PSY20007.
How can I use single subjects to get into a full degree?
First, find the degree that you would like to study on our website.
If that degree allows entry via undergraduate subjects, there will be information about this under the Entry Requirements section. You will find a list of 2-4 open enrolment subjects you need to successfully complete to qualify for admission into that qualification.
Once you pass those subjects, you will satisfy the academic requirements for the degree, and you can apply for entry.
